Emergency #6 – FLASHCARD

26-year-old male chest pain after fall from tree:

What do you see?

(1) Air density in the base of the neck and para-aortic region on the chest x-ray
(2) Confirmation of pneumomediastinum on chest CT
(3) Often due to alveolar rupture, also in deceleration trauma, but check for injuries of airways or upper GI tract
(4) In this case, there was no frank laceration of the bronchial tree on bronchoscopy, and an intact esophagus on endoscopy and esophagram (performed with iohexol)
